AIDS Healthcare Foundation settled federal allegations involving unsupported Medicare Advantage diagnosis codes for $1.44 million
AIDS Healthcare Foundation agreed to pay $1.44 million to resolve Justice Department allegations that its managed-care division submitted or failed to delete inaccurate or unsupported diagnosis codes affecting Medicare Advantage payments from 2017 through 2023. The government said many questioned codes were not removed until 2024 or 2025, after AHF learned of the investigation. The settlement made no determination of liability. DOJ also reported that AHF cooperated, enhanced its compliance program and submitted deletions for codes its medical-record review did not support.
Status matters. This page records a sourced development, not a completed No Halo investigation. An allegation is not a finding. Dismissals, settlements, judgments, corrections and final outcomes remain distinct.
Donor relevance
Why this matters
The settlement puts a defined financial cost and corrective-action record around previously unresolved federal allegations. Donors should weigh both the alleged multi-year coding failures and the documented cooperation and remediation credited by DOJ.
Attribution
Who reported or established this
The allegations, settlement terms and description of corrective action were reported by the U.S. Department of Justice. The underlying whistleblower case was brought by former AHF coder Donna Irons on behalf of the United States.
Evidence limits
What remains unverified
No Halo has not established that AHF intentionally submitted every questioned code, the total value of any resulting excess payments, or the continuing effectiveness of its revised compliance controls. The settlement expressly states that liability was not determined.
Right of reply
Organization response
No separate AHF response was located in the consulted material. DOJ reported that AHF cooperated with investigators, enhanced its compliance program and proactively deleted diagnosis codes that its review found unsupported.
